Cheilitis and stomatitis may have many causes, but allergic contact dermatitis should be included in the differential diagnosis. Reactions to foods, additives, and beverages can be the cause of cheilitis; therefore, food and drink preferences should be queried as part of the patient history. The authors report a positive reaction to sodium metabisulfite — the contact allergen of 2024. This allergen was found in a wine that the patient consumed, and discontinuation of the wine intake resulted in clearance of the dermatitis. Additionally, the authors report a case of stomatitis and eyelid dermatitis found to be secondary to the consumption of cinnamon on apples each morning after identifying cinnamic alcohol as an allergen. Discontinuation of this practice led to the resolution of the dermatitis.
This article reminds clinicians that, when questioning patients regarding exposures, we should also remember to inquire about food and beverage intake, as these exposures may be a cause of dermatitis — especially in cases of cheilitis and stomatitis. Patch testing may help identify the responsible allergens.
