I think there are some missing items in the case discussion. One valuable piece of information would be to know what was the duration between neoadjuvant chemotherapy and date of relapse. To me it seems a bit unclear because the author mentions 5 months ago but does not specify what is the time reference from therapy initiation.

since it is isolated tumor site ,suggest wide excision if feasable followedby capecitabine

I suggest a local treatment such as surgery or radiotherapy and chemotherapy with a platinum salt in the scheme

i like the idea of chemo first followed by restaging to assure that it is still localized; then complete excision if possible; then local RT. After that I would consider capecitabine for six months.
As for the chemo to use- I would probably go with gem/carbo.
Any takers for this strategy?

Consider a right subpectoral mass and local radiotherapy. After offer a chemotherapy schedule with Gemcitabine + Platin.
