I would add Basal insulin and DPP4. For glycolic control and for TG's I will replace Gemfibrogyl with lipaglyn 4 mg

I would add Empagliflozin start with 10 mg OD and after one month increase to 25 Mg OD, also consider adding Semaglutide start with 0.5 Mg Sub Cut weekly and gradually increase as needed can go up to 2 mg
If need we can add sitagliptin
