I had such a case and I did her total mastectomy and the pathology revealed two foci of invasive cancer in the residual breast tissue. I really think in these cases that surgery is a must

Completion mastectomy is the only treatment

She should have total mastectomy followed by radiation therapy to the breast plus axilla, then might consider capecitabine for residual disease and hormonal treatment

Yes, I would perform completion mastectomy. You have multifocal disease which is a clear indication. Is she eligible for a trial? Wonder if anyone's giving palbociclib in the adjuvant setting.

completion mastectomy plus Radiotherapy plus adjuvant hormonal therapy if I get PENELOPE trial may be added palbo additionally she need contrlateral profilactic mastectomy -CHECK 2
