    The life expectancy of a 70-year-old woman can be estimated at more than 10 years. The question is therefore whether the risk that in situ cancer « low grade » to evolve into an invasive lesion is high during this period. Several factors also may be involved in the radiotherapy decision. What is the real risk that the residual lesion is underestimated? In our experience extensive surgery of a unique positive margin will be negative. It is not known whether the initial lesion was expressive with micro calcifications and whether it was readable to the RMI. The reduce risk of recurrence with hormonotherapy in this case is largely equivalent to that of extensive surgery and even radiotherapy. The latter therefore should be discussed according to the choice of the patient. If she is reluctant you could content with a simple strict surveillance accompanied by a sensitive and relevant imagery. (With hormonotherapy).

    Recurrence in DCIS is determined by: age, size and grade. the pt has one bad prognostic sign.Margins being widely neg and the cancer in the rt breast though the patient has coronaty artery dis.radiotherapy followed by hormonal treatment is a good option. An AI might be considered in DCIS
