Association of Neurologic Examinations With Increased Time Spent With the Patient in Ambulatory Care
TAKE-HOME MESSAGE
- This study analyzed 4009 neurologist visits and found that performing a neurologic examination added 4.0 to 7.5 minutes to visit duration. Visits with a neurologic examination averaged 29.9 minutes compared with visits without an examination, which averaged 25.0 minutes. The increase in time was more pronounced for new patients, pain-related visits, and those with known neurological diagnoses.
- This large study assumes that time spent on neurological examination has remained constant over the years and that the quality of neurological care is standardized and uniform among neurologists. The outcomes noted are probably more important for the insight that they provide into how little time neurologists spend on acquiring good neurological histories and performing neurological examinations — interventions that set a visit to a neurologist visit apart from a visit to other specialists evaluating the same condition. Neurological history and examinations remain fundamental steps before an accurate diagnosis and guide diagnostic testing and therapeutic interventions, and a duration of 25.0 to 29.9 minutes seems too short for a good, high-quality history and examinations, assuming no time was spent on counseling and education.
