Treating a Symptomatic Glioma in a Pregnant Patient
A 28-year-old right handed woman with a history of neurofibromatosis type I and a diagnosis of left optic pathway glioma presents for a follow up evaluation. Her brain MRI with contrast shows interval increase of her known L medial temporal expansile lesion with chiasmal involvement on T2-FLAIR sequences compared to her most recent study three months prior. She undergoes a biopsy with pathology being resulted as pilocytic astrocytoma without a KIAA1549-BRAF gene fusion. Post-operatively, her exam is notable for a R superior quadrantanopsia. The patient had been under observation since her diagnosis but given interval increase in tumor size over a short period of time, her treating physicians decide that treatment is warranted. At this time the patient discloses that she is 18-weeks pregnant. How would you manage this patient?
